  • Columellar deviation and shortening — the central pillar of the nose is foreshortened and deviates away from the cleft side, particularly in bilateral clefts where the columella may be markedly short.
  • Septal deviation — the septum is characteristically deviated, contributing both to external asymmetry and to nasal obstruction.
  • Nostril shape and floor deficiency — the cleft-side nostril is often horizontally oriented, with a deficient or absent nasal floor.
  • Functional obstruction — the combination of septal deviation, valve collapse, and a distorted nasal base frequently leaves the airway significantly compromised.

Because the deformity affects every layer of the nose, cleft rhinoplasty must address skeletal base, cartilage framework, lining, and skin together. Correcting the external appearance without addressing the septum and the airway produces an incomplete and unstable result.

Surgical Techniques

Cleft rhinoplasty is staged across childhood and into adulthood, and the techniques used depend on the stage of care. The components below are assembled into a plan specific to the individual deformity, the prior surgery, and the patient’s age and growth status.

Stages of Cleft Rhinoplasty

Primary, secondary, and tertiary correction across the cleft care pathway.

Cleft nasal correction is delivered in stages. Primary correction is performed at the time of the original cleft lip repair in infancy, repositioning the nasal structures as the lip is repaired. Secondary cleft rhinoplasty is undertaken later in childhood or adolescence to address residual deformity once growth allows. The Open Structural Approach

The asymmetry and the effects of prior surgery make direct visualisation of the entire nasal framework essential — the surgeon must see precisely how the cartilages are malpositioned and what structural resources remain before reconstruction can begin.

Alar Base & Nostril Correction

Repositioning the displaced cleft-side nasal base and rebuilding nostril symmetry.

The displaced alar base on the cleft side is repositioned — moving it inward, upward, and forward toward symmetry with the non-cleft side — and the deficient nasal floor is reconstructed where required. Nostril shape and symmetry are addressed directly, recognising that the cleft-side nostril is typically horizontally oriented and asymmetric.

Tip Cartilage Repositioning & Grafting

Restoring projection, definition, and symmetry to the cleft-side tip.

The flattened, malpositioned lower lateral cartilage on the cleft side is repositioned and supported. Structural grafts — including tip grafts and alar contour grafts — are used to restore the projection, definition, and symmetry that the cleft-side tip lacks, and to support it durably against the contractile forces of scarred tissue.

Columellar Lengthening

Addressing the short, deviated columella, particularly in bilateral clefts.

The columella — the central pillar between the nostrils — is characteristically short and deviated in cleft deformity, and markedly so in bilateral clefts. Columellar lengthening and straightening, using local tissue and structural grafts, re-establishes tip support and the central symmetry of the nose.

Septal Correction & Airway Reconstruction

Septal deviation is a consistent feature of the cleft nose and a major contributor to obstruction. Septal correction straightens the deviated cartilage and bone, and spreader grafts and valve support are used to reconstruct the airway. Functional restoration is planned with the same priority as aesthetic correction.

Cartilage Graft Sources

Septal, auricular, and costal cartilage for structural reconstruction.

Cleft rhinoplasty is graft-dependent. Septal cartilage is used where it remains available; auricular (ear) cartilage provides an excellent secondary source for softer structural grafts; and costal (rib) cartilage provides the abundant, reliable material required for the most complex reconstructions where prior surgery has depleted the septum. Recovery & Aftercare

Recovery from cleft rhinoplasty resembles that of revision surgery — more prolonged than primary rhinoplasty, because scarred and previously operated tissue swells more and resolves more slowly. Dr Sparks prepares patients and families thoroughly for the recovery process.

First 2 Weeks

Splint, dressings, and the most pronounced swelling.

  • A splint and dressings are typically worn for 10 to 14 days — longer where rib cartilage grafting has been performed.
  • Swelling and bruising are most pronounced in the first two weeks; where rib cartilage has been harvested, chest-wall discomfort is also expected and managed.
  • Most patients return to non-strenuous school or work within two weeks, depending on the extent of the procedure.
Weeks 2 to 12

Gradual resolution of swelling and early functional improvement.

  • Significant improvement in swelling by 4 to 6 weeks, though scarred cleft tissue resolves more slowly than an un-operated nose.
  • Airway improvement, where functional work was performed, is often noticed relatively early.
  • Light exercise typically resumes after two to three weeks; strenuous activity and contact sport are deferred for at least eight weeks.
Months 3 to 18

Slow settling and the final result.

  • Continued settling of the tip and nasal base over 12 to 18 months.
  • The settled outcome is not fully apparent until 12 to 18 months post-operatively in most cleft cases.
  • Long-term review confirms the structural durability of the reconstruction and informs any further staged care.

Risks and Important Information

  • Adverse reaction to anaesthesia or post-operative thromboembolic events.
  • Asymmetry, under-correction, or over-correction requiring revision surgery.
  • Outcomes that fall short of expectations despite a technically appropriate procedure.
